　　目前新型冠状病毒性肺炎疫情正在蔓延，防控形势依然严峻。外省返村人员探亲过年结束，返程管理成为当前最为重要的任务，为切实管控好返程人员的自身安全，应对可能发生的情形，织密疫情防护网，既实现返程人员回城工作的现实需求，又要保证可能发生的疫情的可管可控，全力维护好人民群众的利益。
　　返程工作人员，应提供：
　　1、在家医学观察期满的书面证明。
　　要求：以其返村车票上到**的日期为准，由村卫生室提供。（某人，男，年龄，住址，联系电话，某月某日至某日在其家中隔离观察，期满**天，期间监测其体温正常，未有感冒、呼吸系统症状。某村医生：某某，日期。）
　　2、提供与其本人密切接触人的详细情况资料。（包括姓名、性别、年龄、住址，与其亲属关系，联系电话）
　　3、提供“其一直在家隔离观察，未外出活动，有无接触过武汉疫区的发热病人，有无去过武汉疫区旅行或居住，有无接触野生动物史。如有隐瞒情况，造成影响的，自愿承担由此带来的责任”等保证材料。
　　4、提供其返回单位的信息资料。包括的详细地址，单位完整名称，单位领导姓名，联系电话等内容。
　　5、提供其拟返程的日期、路线，目的地及车票车次箱号机次，上车时间，站次等资料。
　　6、村组织提供其在家医学观察期间的每日的体温监测记录资料，盖章，村干部签字。
